Please make sure you arrive about 10 minutes early to have enough time to fill out a New Patient Form.

Your Initial Examination

Your dentist will begin by reviewing your medical and dental history and conducting a thorough dental exam. Your exam includes an examination of your gums, teeth, bite and jaw, an oral cancer screening and any necessary X-rays.

Your Treatment Plan

Your dentist will develop a long-term dental treatment plan based on your history and examination. Your dentist will also take the time to answer any questions you have about your oral health and treatment options.

*Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.

  To schedule an appointment at LightHouse Dental Chatham, please contact our office during our regular hours of operation.

• How do I cancel or reschedule an appointment?

  To cancel or reschedule an appointment, please give us a call. Please let us know about any change to an appointment as soon as possible so that we can offer the space in our schedule to another patient.

• Do you offer appointment reminders?

  In the days leading up to your appointment, you will receive automated phone, text or email appointment reminders from us. If you need to cancel or rebook an appointment, please call us as soon as you can and let us know.

• Do you provide services in any language other than English?

  Our office can provide dental services in several languages including Spanish, French and Punjabi.

• Does your clinic have on-site parking?

  Yes, we have free on-site parking available for all our patients. We are also accessible by public transportation.

The full payment for treatment must be paid at the time of your appointment. We accept payment by debit, credit card or e-transfer.
